- Post-Graduate Degree/Diploma Certificate.
- Professional Publications in an indexed National as per MCI standard Medical Journals.
- Experience Certificate wherever required.
- Date of Birth Certificate.
- Domicile/ Non-Domicile may also apply subject to the condition that preference will be given to the Domicile candidate.
- Any other certificate pertaining to the Speciality.
- Affidavit duly attested from Ist class Magistrate stating that the doctor is not in active Govt Services.
